You must send us the following:

* A letter from your Consultant stating your Medical history and that you are fit to travel.
* Two copies of an up to date report on HIV, Hepatitis B and C status
* An E111 form, if you are an EC Citizen.
* Dialysis details, type of Dialyser, Heparin Etc.
* Medication : if on Erythropoeitin, you must bring your own supply.
* On receipt of the above items, we will then confirm a Dialysis Holiday
